These five centers are providing primary health care to the medically underserved in Autauga County, Chilton County, Elmore County, Lowndes County, and Coosa County.

Lister Hill Health Center (LHHC) is a large freestanding operation that serves as our
home base for administrative purposes. This center offers the same services as our
other locations with the exceptions of the OB/GYN Department. The departments are
supported by in-house lab, x-ray and pharmacy. Physicians as well as certified
registered nurse practitioners make up the medical provider staff. The nursing staff
consists of RNs, LPNs, and medical assistants. There is also an RN case manager for
diabetics, and RN health educator for OB services. Assisting with specialty referral
tests, surgery, mental health and social needs are social workers. As case manager
provides assessment and follow up services to OB patients. Extended hours for
working clients are scheduled on Tuesday from 4:30 - 8:30 PM at the LHHC only. An
immunization clinic for pediatric patients is also provided at that time.

Dental services including preventative, restorative and limited emergency care are also provided. A Women's, Infants, and Children (WIC) nutrition program provides nutritional counseling and vouchers to pregnant mothers and children who qualify. Transportation services are provided to assist those patients who have no other source.
LHHC sits adjacent to a large public housing complex and provides immediate walk-in access to residents. However, people from all areas of the city and other counties find their way to LHHC primarily because of its long history in providing quality health care to residents of all ages.

The Ramer Health Center (RHC) is a freestanding facility serving the needs of rural
areas of south Montgomery County and the immediate areas of adjacent counties. RHC
is essentially a solo medical practice. The medical provider brings a unique blend
of skills to the practice. The provider is capable of seeing a full range of
clinical issues presented in the life cycle. In addition to having on-site lab and
x-ray, RHC has a fully equipped dental clinic that provides dental services on a
routine basis. Podiatry is also offered on a weekly basis. The availability of a
small formulary is designed to initiate medications; however, courier services from
LHHC provide next day service for prescriptions that are filled at LHHC. Continuity
of care for RHC patients is facilitated through social services, lab, and the
professional staff at LHHC.

Given the high incidences of diabetes in the population, the RN diabetic counselor from LHHC conducts an exercise and nutrition program weekly that consists of a walking routine and a review of various diabetic issues of interest to the participants.

Montgomery Primary Health Care Center (MPHCC) offers a unique setting for delivery
of primary care service. It is not freestanding; it occupies the ground floor of the
newly constructed Montgomery County Health Department (CHD) facility. The location
offers ready access to the west side of Montgomery and particularly three large
public housing complexes, two of which are within a half mile, and others within 1
1/2 miles of the center. Under this arrangement, residents of the area are able to
receive traditional public health services and primary care services under the same
roof. The referral agreement between the two entities encourages MPHCC to refer
immunizations, lead and TB screens and WIC clients to the CHD. The CHD refers
persons needing medical treatment and follow up. MPHCC is our second largest center
and offers essentially the same schedule of services as if offered by LHCC in
addition to OB/GYN.

The Chisholm Family Health Center (CFHC) was originally designed as a school-based
clinic in the Chisholm Elementary School. However, the demand for care from parents
and siblings soon dictated that consideration be given to a different approach for
serving the Chisholm area. The city agreed to lease a building located adjacent to
the school. Currently, the center is a freestanding facility that continues to focus
on pediatric care but provides adult medicine two days a week. The provider staff
consists of a full-time pediatric CRNP and a half-time family practice CRNP whose
preceptors are physicians from LHHC and MPHCC. The center offers limited lab
services but relies on both MPHCC and LHHC for continuity of care especially for
prescriptions, x-ray, lab and social services.

The Family Health Center is located in Tyler, Alabama and it has been at its current
location on US Highway 80 for the past four years. The center offers services to
both adults and children of all ages. The scope of care for this center is the same
as our other locations with the exception of the OB/GYN Department. This center
provides patient and clinical services for the residents of Tyler, Lowndes and
Montgomery Counties. It is in the same community as the White Hall Elementary School
making it accessible to school children all year long.

Our Crossroads Healthy Lifestyle and Wellness Center, which opened July of 2003,
is HSI's direct response to Healthy People Objectives 2010, by providing exercise and
fitness equipment and wellness seminars that address diabetes, weight management,
cardio-fitness, stress, and smoking cessation. The Lifestyle Center is located on
Decatur Street in Montgomery.
