As a patient of Health Services, Inc., you are entitled to certain rights and privileges, which are referred as your "Patient's Bill of Rights." Included with your Bill of Rights are Patient Responsibilities.
The following are your RIGHTS:
- Confidentiality It is the policy of Health Services, Inc. (HSI) to offer the utmost confidentiality to all aspects of the patient's visit to the health center.
- Privacy HSI strives to offer maximum privacy for patients visiting the facilities.
- Security Security for person and property will be assured through the organization's comprehensive security plan.
- Communication about Patient Rights Patients will be made aware of their rights through the use of brochures in the patient waiting areas and at the time of registration.
- Reasonable Access to Care Improving access to health care services is central to the mission of HSI. Hours of services are arranged to coincide with community needs.
- Resolution of Complaints Complaints from patients regarding the services provided by HSI centers will be received by any staff member, and forwarded to Administration for resolution and follow-up.
- Considerate Care that Respects Patients' Personal Values & Beliefs Every effort will be extended by HSI's staff to be respectful of the personal values and beliefs of our patients.
- Pain is Assessed in all Patients In the initial assessment, the organization identifies patients with pain. When pain is identified, the patient can be treated within the organization or referred for treatment.
- Designation of a Surrogate Decision-maker Patients who feel unable, or in other ways, not prepared to make critical health care decisions have a right to designate a surrogate health care decision maker.
- Information about Fee Schedules and Payment Policies A schedule of fees is filed with the U.S. Public Health Service annually, and is a public document. Patients may review this information at any time.
- Making Informed Care Decisions It is the policy of HSI to inform patients regarding the risks, options or alternatives to the suggested treatment.
- Considering Ethical Issues that Impact Care Ethical issues related to patient rights and responsibilities.
- Resolving Conflicts about Care Decisions It is the fundamental right of the patient to be in agreement with the proposed plan of treatments.
- Making Care Decisions During Investigative Studies Currently, HSI does not conduct investigative studies.
Your responsibilities include the following:
- Keeping your appointment You are responsible for keeping appointments and for notifying the center in advance when you are unable to keep appointments.
- Providing information The patient is responsible for providing, to the best of his or her knowlege, accurate and complete information about present complaints, past illnesses, hospitalizations, medications, and other matters relating to his or her health. The patient and family are resonsible for reporting unexpected changes in the patient's condition. The patient and family can help the organization improve its understanding of the patient's environment by providing feedback about services, needs and expectations.
- Asking questions The patient is responsible for asking questions to clarify instructions for their care.
- Following instructions The patient and family are responsible for following the care, service, or treatment plan developed. They should express any concerns they have about their ability to follow and comply with the proposed care plan or course of treatment. Every effort is made to adapt the plan to the patient's specific needs and limitations. When such adaptations to the treatment plan are not recommended, the patient and family are responsible for understanding the consequences of the treatment alternatives and not following the proposed course.
- Accepting consequences The patient and family are responsible for the outcomes if they do not follow the care, service, or treatment plan.
- Following rules and regulations The patient and family are responsible for following the organization's rules and regulations concerning patient care and conduct.
- Showing respect and consideration Patients and families are responsible for being considerate of the organization's personnel and property.
- Meeting financial commitments The patient and family are responsible for promptly meeting any financial obligations agreed to with the organization including:
- providing legitimate proof of eligibility for any third-party program (Medicaid, Medicare, private insurance, etc.)
- assuring that only authorized signatures are used in applying for health care.
- assuring that minor children are always accompanied by an authorized person when they come to HSI for medical care.
- being responsible for paying for services at the time they are rendered, including all known third-party co-payment requirements. Health Services, Inc., will work directly with you to assure a plan for full payment of your obligations.
